REGISTRATION DATES AND TIMES:

Registration will also be at the In-House Tournament June 4th/sup>,
5th and 6th at Kyte Monroe.

WHERE:
Registration will be held at the Civic Arena, located on Stephens (9 ˝
Mile) between Little Mack and I-94.
COST:
$75.00 for the
first child, $65.00 for additional children from same family./p>

ITEMS NEEDED:
Photograph – (will be stapled to registration)/b>
Birth Certificate for Age
Verification (will be returned once age is verified) if haven’t played with us
in the past.
Check, Cash or Money Order
made payable to AYSO Region 158
VIP LEAGUE (Very Important Players)
VIP League is available for special needs players ages 5 and
older.

available.
If you would like to pre-register your player you may do so online at
www.eayso.org. You will still need to attend one of the registration dates, but this
method will save you a lot of time.
